DEVB TC(W) No. 8/2025, Annex A 7.5.1(iii)
DWSS site safety inspection records module
The Site Safety Inspection Records module is the third mandatory DWSS module in DEVB TC(W) No. 8/2025 (Annex A 7.5.1(iii)). It records weekly and monthly safety walks, environmental inspections and safety performance, with photos and a full non-compliance cycle. Kodifly delivers it in SpatialSense.
What the module covers
Annex A 7.5.1(iii)(a) gives the module two parts. The first is safety inspection checklists, recording results and photographs of weekly and monthly safety walk inspections and environmental inspections, with reference to Chapter 8 of the Construction Site Safety Manual. The second is safety performance records, covering accident rate statistics, LD/MD improvement and suspension notices, conviction records and other information, with reference to the Contractor's Monthly Report on Safety Performance proforma in Chapter 12.
In the DWSS filing format the records use the type code SSR. Sub-types include PW (pre-work safety check), WSI (weekly safety walk), MSI (monthly safety walk), EI (environmental inspection), PTW (permit to work), NCR (non-conformity report), SPP (safety performance report) and SCR (safety score card) (7.3.4).
What each inspection record holds
Annex A 7.5.1(iii)(c) lists six items the form must be set up to record.
| Record item | What the form holds |
|---|---|
| Who and when | Date, time, and the names of reporting officers and responsible persons |
| Where | Work location |
| What | Description of the works |
| Evidence | Site photos before and after inspection |
| Findings | Logging of non-compliance and remedial works |
| Cycle | Relationships kept across the whole inspection and non-compliance report cycle |
Workflow and who signs
Annex A 7.5.1(iii)(b) sets four stages. The workflow agreed with the employer's representative, within six weeks of commencement, names the people (7.4.4).
Submit
The inspecting officer completes the checklist with photos on the app or browser and submits it (7.4.5).
Review
The reviewer is notified through the workflow and can raise a non-compliance item (7.1.3).
Confirm
The responsible party confirms the finding and the remedial works. Each action is time-stamped (7.3.8).
Complete
The inspection closes once the cycle is done. The record is locked and kept linked to its non-compliance reports (7.3.7).
Statistics the module must show
Annex A 7.5.1(iii)(d) lists nine. The DSR (section 4.5) sets what goes to iCWP.
| Annex A statistic | What iCWP receives (DSR 4.5) |
|---|---|
| Overdue actions, and completed, incomplete or outstanding inspections | Number completed and incomplete in a period |
| Non-compliance, observation, near-miss and incident reports | Non-compliance, near-miss and incident report numbers |
| Accident rate | Accident rate in a period |
| Safety convictions | Number of safety convictions in a period |
| Unsafe acts and unsafe conditions at each inspection | Not listed separately in DSR 4.5 |
| Process time, not in the Annex A list | Average elapsed time, and late rectifications |

Site safety inspection records, answered
What do the safety records have to include?
Safety inspection checklists for weekly and monthly safety walks and environmental inspections, and safety performance records for accident rate, LD/MD notices and convictions (Annex A 7.5.1(iii)(a)).
How is a non-compliance tracked?
The form logs non-compliance and remedial works and keeps the relationships across the whole inspection and non-compliance report cycle (7.5.1(iii)(c)).
Does it link to the Smart Site Safety System?
Where a contract adopts both, the central platform of the Smart Site Safety System is amalgamated into the DWSS as an integrated Smart Construction Management hub (paragraph 7, footnote 1).
Which safety forms have type codes?
Sub-types include PW, WSI, MSI, EI, PTW, NCR, SPP and SCR (7.3.4). They cover pre-work checks, weekly and monthly walks, environmental inspection, permit to work, non-conformity reports, safety performance and score cards.
What does the module send to iCWP?
DSR section 4.5 lists completed and incomplete records, non-compliance, near-miss and incident reports, accident rate, safety convictions, average elapsed time and late rectifications.
Who signs a safety inspection?
Annex A does not name signatories. The workflow agreed with the employer's representative sets who reviews and confirms, and each step is time-stamped with the user (7.4.4, 7.3.8).
